CV Template

The Nominations Service CV template provides a guide to the information we would like to receive from you. There is no need to complete this particular template as long as the CV you submit contains the key information required:

• employment history (including achievements and responsibilities),
• appointments to government and private boards,
• voluntary roles in community organisations,
• membership of professional organisations,
• awards, and qualifications.

All information should be dated by year (with start date and, if finished, end date).

Please also provide all current contact details: postal addresses, home phone number, work phone number and DDI, home and work email addresses, and cell phone numbers.
